NCPPC’s Perinatal and Infant Community Health Collaborative (PICHC) staff provide support, education and outreach to all parenting people before, during, and after pregnancy throughout Jefferson, Lewis and St. Lawrence Counties.

Home Visiting
NCPPC’s perinatal and infant health staff provide free services throughout Jefferson, Lewis and St. Lawrence counties, known as our Community Health Worker (CHW) program. We offer education on topics that may impact general health care, reproductive health and birth control, pregnancy or infant care, healthy pregnancy tips, Safe Sleep, and Shaken Baby Syndrome.
The CHW program offers peer support before, during, and after pregnancy to promote overall health and wellness. Community Health Workers link individuals with health care providers, social services, and community resources to improve access to services and coordination of care.
Our staff work with families to achieve goals created by families and at their pace.
Breastfeeding Support
Cribs for Kids
NCPPC’s Cribs for Kids and Safe Sleep program provides resources and education to new parents on the importance of safe sleep practices. In addition to educational tools, qualifying families will receive a Pack ‘n Play® portable crib if they cannot otherwise afford a safe sleeping environment for their infant.
To be eligible for this free program, participants must agree to the educational requirements as outlined by the NCPPC. The Cribs for Kids and Safe Sleep program is only available to children weighing 30 pounds or less.

Infant Weight Checks
NCPPC provides infant weight checks in the home. This program provides peace of mind to families with concerns about weight gain for their child. We work with pediatricians and other providers to ensure adequate growth and development for infants.
Light Touch Program
NCPPC, in partnership with Canton-Potsdam Hospital, provide four virtual visits to every birthing family. This program screens for post-partum depression, provides support and connection to local services, as well as breastfeeding and other maternal and infant health resources. The CHWs are accessible where you live across St. Lawrence County. 100% virtual, free and confidential!
