Community Health Worker Supervisor Position – Full Time

The North Country Prenatal/Perinatal Council is seeking a full time Community Health Worker Supervisor. This position is responsible for providing supervision and support to a team of Community Health Workers (CHWs) working within Jefferson, Lewis and St. Lawrence counties to outreach and support hard to reach community members and engage them in appropriate health services.

 

Responsibilities:

Provide 1:1 reflective supervision for the CHWs.

Support CHW team to conduct basic health assessments, identify needs, and provide basic health information.

Make appropriate referrals to preventive and primary health care services through home visits and street outreach.

Plan and coordinate outreach, supervise individual and family assessments, assess training needs and provide training for CHWs as needed.

Facilitate case conferencing with CHW team to discuss client cases and provide support.

Develop, in conjunction with the MICHC Program Director and Population Health Director, an overall work plan, including outreach strategies.

Collect data and create quarterly and yearly reports or as required by contract.

Qualifications:

Licensed Social Worker (LMSW or LCSW) or Nurse (BSN) required with valid NYS license.

2 years’ experience providing clinical, 1:1 reflective supervision preferred with knowledge of home visiting preferred.

Must be skilled at supervising, counseling, teaching, and motivating staff.

Must have outstanding oral, written and interpersonal communication skills.

Computer literate; proficient with Microsoft Office.

Familiarity with the geographic area and the population to be served.

Be available on an “on-call” basis to provide consultation to CHWs working with families in crisis.

Bilingual applicants will be given preference.

Personal vehicle and valid driver’s license required.

Director of Population Health

The North Country Prenatal/Perinatal Council, Inc., is seeking a full-time Director of Population Health to coordinate our tri-county community health worker program(CHW) for clients with chronic disease. NCPPC’s Population Health program is intended to reduce emergency room visits and unnecessary hospitalizations by addressing the social determinants of health including poverty, housing security and safety, transportation, access to and food security, and various health care issues. The program provides non-clinical home visits to individuals with a chronic condition to assess and address issues impacting their overall health. The CHWs work on goal setting, problem solving, skill building, and efficacy with individuals to empower them to take charge of their health and wellness with the direct support from the director and supervisor.

Qualifications for this position include a Bachelor’s Degree in social work, education, community health or a field directly related to population health and chronic disease management. The ability to work independently; strong computer literacy skills; excellent interpersonal and organizational skills, with exceptional attention to detail are also required. Preference will be given to those that have three years or more experience working with the target population. Knowledge of local community resources required. Must have valid driver’s license and reliable transportation. American Sign Language or Bilingual speakers will be given preference.
