The Population Health program offers home visiting services to adults 18 years of age and older that have been diagnosed with a chronic illness.  This community health worker (CHW) program provides peer support through home visits to improve health literacy, patient care, and chronic disease self-management.  Community health workers motivate and assist individuals with health care, social services, and community resources to improve access to community services and coordinate care through primary care providers.  The community health worker works in collaboration with the client to ultimately improve the quality of health independently while reducing Emergency Room visits, admissions, and re-admissions to the local area hospitals.


How do you get involved in the program?

  • First, you must be 18 years old or older and have a chronic condition.
  • Reside in Jefferson, Lewis, or St. Lawrence Counties.
  • There is a referral form that is easily filled out and you can access it directly through our website.


Who can make a referral?

  • Doctors, nurses, case managers, social workers, etc., and you can fill one out for yourself!


How it works:

  • A Community Health Worker meets with a client at a mutually agreed location or in the home.  They work as a wellness coach and offer support and motivation to obtain healthier goals.  They work with their client to achieve more independence within their health management.

A free and voluntary service to
expectant and new parents.


Supporting Families Right From the Start:

Healthy Families is a free service that partners expectant and new parents with knowledgeable and caring Support Specialists to help grow children who are happy, healthy, safe, and smart!  The partnership can begin during pregnancy and last until the child reaches school age.  Services include helping families access community resources and services; offering families information on their developing baby, newborn, and growing child; parenting; ideas for play; health and safety education; connecting families with medical providers; and assessing children for developmental delays.

In-home support can change the lives of children and their families.  Research shows Healthy Families New York (HFNY) works for many families, from first-time teen parents to families with other children. Families have healthier babies, demonstrate better knowledge of parenting and child development, create positive family bonds, develop connections to community services, and have children who do better in school.


How Healthy Families Works:

The Healthy Families program offers in-home services to support expectant families and new parents with the changes and needs that often come with the birth of a new child. Services are voluntary and provided at no cost.

The first step...

Anyone can make a referral to Healthy Families; you can even refer yourself!  We strive to talk with all expectant parents and parents of newborns in the communities we serve.  Our goal is to identify strengths and challenges prenatally or at birth and provide appropriate information and referrals to help babies get off to a healthy start.  Complete the survey here to get started: http://bit.ly/hfjefferson.


What's next...

A specially trained staff member may meet with you to further explore the strengths and stressors that are unique to your family.  After this discussion, you may be provided with referrals to services in your community that can help you meet your needs and goals.


On-going support...

A Family Support Specialist (FSS), hired from and representing the language and culture in your community may offer on-going support.  This FSS partners with you to support you right from the start.  They work with you and your family in your home environment.  Home visits often begin prenatally and may be offered until your child is in school or Head Start.  They may be provided on a weekly basis at first but decrease over time.


The FSS can support your family by...

  • Connecting you with medical providers for prenatal/well-baby visits and immunizations.
  • Providing information on prenatal care, parenting, infant and child development, and child health.
  • Demonstrating activities for you to practice with your child to increase bonding and to stimulate cognitive and physical development.
  • Assessing your child for developmental milestones and providing referrals for early intervention if needed.
  • Helping your family access community resources and services, such as job training, ESL classes, legal services, GED preparation classes, and health care.
  • Providing opportunities for you and your family to attend social and educational events.  These events enable parents to establish connections with other parents of young children and share parenting experiences, successes, and challenges.

From October 1st through October 25th, the North Country Prenatal/Perinatal Council will be highlighting Agency programs that strive to make a difference in Jefferson, Lewis and St. Lawrence Counties.
