Nutrition Outreach and Education Program (NOEP) Coordinator

 

The North Country Prenatal/Perinatal Council is seeking applicants for Lewis County Nutrition Outreach and Education Program (NOEP) Coordinator. The NOEP Coordinator assists income eligible households with completing the SNAP (formerly food stamps) application process, including gathering supporting documentation. Travel to outreach locations, such as senior centers, food pantries, and human service agencies across Lewis County, to assist with applications, conduct educational presentations and targeted outreach campaigns about the benefits of SNAP is required. Home visits may be required in order to assist Lewis County residents in the application process.

Candidates must have two years of experience in consumer relations, human services or related experience; associates degree in related field (appropriate work experience will be considered in lieu of education) is preferred.

Must be creative, detail-oriented, able to work independently, possess good public speaking skills, and be computer literate. A valid driver’s license and reliable transportation is required.

Community Health Worker/ Per Diem in Jefferson and Lewis Counties and in the Gouverneur and Potsdam areas is St. Lawrence County

 

The North Country Prenatal Perinatal Council, Inc. is seeking Community Health Workers to improve population health in targeted communities across Jefferson, Lewis, and St. Lawrence counties.  CHWs will coordinate direct services and provide healthy lifestyles support to individuals with chronic health issues or in need of additional help to avoid re-hospitalization or emergency room use. Supports may include: assistance with developing a wellness plan, education, assessment of barriers to healthy living, access to health insurance and insurance literacy. Principal responsibilities include participant recruitment, home visits promoting wellness, referral to community resources and programs, coaching, and other individualized supports. Qualifications include: high school diploma or equivalent, regard for patient privacy, reliable transportation, good communication skills, experience with data management, and a flexible schedule. Bilingual or American Sign Language is preferred.
