Family Support Worker

The North Country Prenatal/Perinatal Council, Inc. is seeking a full-time, non-exempt, Family Support Worker for our new Healthy Families of Jefferson County case management and home visiting program.  Our Healthy Families program will match parents with knowledgeable and caring Family Support Workers who provide long-term case management and parent education services to prenatal and parenting women with children ages 0-5 (as per Healthy Families NY guidelines and standards).  Services include helping families access community resources and services, educating parents on parenting techniques and child development, connecting families with medical providers, and assessing children for developmental delays.

The Family Support Worker is responsible for initiating and maintaining regular contact with families primarily in their homes, which will include working flexible hours to the extent necessary to accommodate the schedule of the families.  The Family Support Worker will complete timely documentation of all home visits, case notes, and other program documentation as per agency and program guidelines in both written and electronic format.  Knowledge of community resources and strong communication (verbal and written) skills are required.

The successful candidate is required to have a minimum of a High School Diploma/GED with 1-3 years of experience working with families in a community setting.  An Associate’s Degree in Human Services or related field with prior parenting and/or child care experience is preferred.  The selected candidate must have a current NYS driver’s license that is clean and valid, a reliable personal car to carry out employment duties, and an active auto insurance policy.  Selected candidate must clear a criminal and child abuse/neglect background check for employment.  An indigenous community resident or long-term employee of a human services provider serving Jefferson County communities is preferred.  Candidates who are bilingual in Spanish and/or American Sign Language will be given preference.

Community Health Worker/Per Diem – 35 to 50 hours per month

The North Country Prenatal Perinatal Council, Inc. is seeking Community Health Workers to improve population health in targeted communities across Jefferson, Lewis and St. Lawrence counties. CHWs will coordinate direct services and provide healthy lifestyles support to individuals with chronic health issues or in need of additional help to avoid re-hospitalization or emergency room use.  Supports may include: assistance with developing a wellness plan, education, assessment of barriers to healthy living, access to health insurance and insurance literacy. Principal responsibilities include participant recruitment, home visits promoting wellness, referral to community resources and programs, coaching, and other individualized supports. Qualifications include: high school diploma or equivalent, regard for patient privacy, reliable transportation, good communication skills, experience with data management and a flexible schedule.  Bilingual or American Sign Language is preferred.
