Population Health Program Supervisor

 NCPPC is seeking a Population Health Program Supervisor to coordinate our new tri-county community health worker/patient navigation program for clients with chronic disease. Ideal candidates will be highly motivated, organized, possess strong communication skills and interested in improving the health of our tri-county community.

Qualifications for this position include a Bachelor’s Degree in public health or human services administration or a field related to population health and chronic disease management with two years’ experience; ability to work independently; strong computer literacy skills; excellent interpersonal and organizational skills,with exceptional attention to detail. Must have valid driver’s license and reliable transportation. American Sign Language or Bilingual speakers will be given preference.

Community Health Worker-Part-Time (20hrs/week)

The North Country Prenatal Perinatal Council is seeking a part-time (20 hours/week) Community Health Worker (CHW) responsible for identifying high risk women of child bearing age throughout Lewis County. The CHW will assist these individuals in gaining access to health insurance, primary and preventive health services including prenatal care, WIC and other essential and supportive services as needed; provide home visiting services to include but not limited to, health and family assessment and reassessment, education curriculum on health related issues of pregnancy, child development screenings, and referrals where needed; manage a caseload by keeping an accurate case record of each participant; attend outreach activities, facilitate education and/or support groups/ workshops, and perform other related duties as needed.

High School diploma or equivalent required. Some college or experience in the Human Services field a plus. Must have a current, valid driver's license and a personal car to carry out all employment duties and active automobile insurance policy required. Knowledge of community resources required. Must have basic computer skills. Strong interpersonal and communication skills required. Seeking indigenous community resident or long term employee of the targeted communities. Bi-lingual candidates will be given preference. Ability to work flexible hours, including some evening and weekend hours on occasion.

Community Health Worker/Per Diem – 35 to 50 hours per month

The North Country Prenatal Perinatal Council, Inc. is seeking Community Health Workers to improve population health in targeted communities across Jefferson, Lewis and St. Lawrence counties. CHWs will coordinate direct services and provide healthy lifestyles support to individuals with chronic health issues or in need of additional help to avoid re-hospitalization or emergency room use.  Supports may include: assistance with developing a wellness plan, education, assessment of barriers to healthy living, access to health insurance and insurance literacy. Principal responsibilities include participant recruitment, home visits promoting wellness, referral to community resources and programs, coaching, and other individualized supports. Qualifications include: high school diploma or equivalent, regard for patient privacy, reliable transportation, good communication skills, experience with data management and a flexible schedule.  Bilingual or American Sign Language is preferred.
